Request for Proposal: Coordinated Intake Services

The Northeast Florida Healthy Start Coalition is seeking qualified providers to implement Coordinated Intake & Referral  services in Baker, Clay, Duval, Nassau and St. Johns counties aimed at addressing the region’s infant mortality rate.   Funding of up to $450,050 will be available for implementation of this initiative during the project period (beginning July 1)

This Request for Proposal (RFP) is open to all qualified providers, including public, non-profit and for-profit organizations. Preferred applicants will be medical facilities such as OB/GYN offices or hospitals. A letter of intent is required will be accepted ongoing. Full applications are also accepted on an ongoing basis.

The Coalition’s Contracts & QI Committee will evaluate all applications with final approval from the Board of Directors.
