REQUIREMENTS

This position requires that you have a four-year college degree in one of the following areas: Social Work, Psychology, Community Health, a health-related field such as nursing, health education, health planning or health care administration, or Social Services OR a two-year college degree WITH a minimum of three years’ experience in public health/community development OR licensure as a Licensed Practical Nurse WITH four years of public health/maternal child health experience. Must be willing to use your personal vehicle and travel to meet with clients in their homes or at other designated places in the community as needed. Must have at least 6 consecutive months of experience with case management OR have you completed an internship of at least 3 months’ duration that included case management. Must be able to lift up to 15 pounds unassisted. Must be able to walk on uneven terrain and climb stairs.

Working knowledge of Microsoft Office Suite and experience documenting case notes electronically is preferred.

The incumbent an employee of the Florida Department of Health in Clay County works to protect, promote and improve the health of all people in Florida through integrated state, county, and community efforts. Perform ongoing risk assessments, developmental screening, social service evaluations and counseling to Healthy Start clients and their families. Participate in the direction of the interdisciplinary team development of Family Support Plans for select Healthy Start participants through ongoing care coordination. Provide/deliver social and case management services to clients in the home, community and clinic as needed. Document aforementioned services in accordance to Healthy Start Standards and Guidelines.

Perform direct care and consultative services to enhance the participant’s ability to access care. Explore local resources and meet with alternative service delivery providers to promote access to health care through community agency collaboration.

Serve as a social work Healthy Start team case manager, providing services to at risk pregnant women and infants, with particular emphasis on developmental screening, interdisciplinary approaches to care, outreach activities, intervention strategies and care coordination.

Coordinate activities and provide human service related training for student interns, volunteers and paraprofessional resource mothers and fathers to assure the delivery of quality social services and coordination with other service providers (as available per local resources/funding).

Actively participate in weekly Case Management meetings, monthly record reviews, and annual Coalition site visits. Prepare reports for Supervisor or designee to the implementation of social services/ case management duties. Participate in the formulation of programs and policies as they relate to the provisions of social services and prenatal/postnatal Case Management.

Conduct group sessions for pregnant women and parents of infants in need of emotional support and parenting skills training to reduce risks associated with infant mortality and morbidity (as available per local resources/findings). Plan, develop, coordinate and conduct training sessions, workshops and meetings for professional staff and local community agencies regarding implementation of outreach, care coordination and social service intervention strategies.

Cultivate and maintain professional linkage with colleagues in the communities and state agencies. Represent the CHD at local meetings and state conferences, coalitions, workshops and seminars regarding assigned area of responsibility.

Perform other duties as assigned/required by the position and within the realm of the classification specifications for a Social Services Counselor.
